The problem
Executive health screens were designed to reassure. Not to act.

You receive a 20-page report. Most results are "within normal range." The report goes into a drawer. Nothing changes.

What the report doesn't tell you: your fasting insulin has been rising for eight years while your HbA1c stayed normal. Your ApoB particle count puts you in the top decile of cardiovascular risk despite a textbook cholesterol panel. Your testosterone is technically adequate, but your SHBG has doubled since your thirties, and what's biologically available is a fraction of what the number suggests.

These are not obscure findings. They are the difference between intervening now – when the range of options is wider – and discovering the problem a decade from now, when it may not be.

The proposition

This is not a health screen. It is an ongoing medical advisory relationship with a Consultant physician who holds your entire clinical picture and meets with you every two weeks to refine it.

Your advanced blood panel, DEXA body composition, cardiopulmonary exercise test, biological age assessment, continuous glucose monitoring data, nutrition audit, and wearable output are not reviewed in isolation. They are read together, by the same physician, every time – because the value is in the pattern, not the individual number.

Where your clinical data indicates a need for specialist input – a personal trainer, a dietitian, a therapist – the direction comes from this integrated picture. You are not assembling your own health team in the dark. You have a medical strategist who sees the complete map and coordinates accordingly.

Between consultations, you have priority access to your physician via encrypted messaging. A question at 11pm on a Tuesday doesn't wait until your next appointment. It gets a considered response from the person who holds your full clinical picture.

Every retainer engagement begins with a comprehensive baseline across nine investigative domains. Results are repeated at defined intervals – not annually, but at three- to six-month milestones – so that trends, not snapshots, drive your strategy.

60+ biomarkers across 13 physiological domains. Fasting insulin, ApoB, Lp(a), full thyroid including free T3, sex hormones with SHBG-corrected free testosterone, IGF-1, active B12. Repeated at every milestone.

Regional lean mass, visceral adipose tissue, bone mineral density. The objective measure of whether your training and nutrition strategy is actually working – independent of the bathroom scale.

VO₂max, ventilatory thresholds, cardiac output under maximal stress. The single strongest predictor of all-cause mortality in the exercise physiology literature. Subject to medical clearance.

Free onboarding videocall
A 30-minute conversation to establish scope, confirm suitability, and answer your questions. There is no obligation and no fee. This is also where I explain clearly what this service is and what it is not.

Clinical intake and baseline investigations
A digital consent agreement, a comprehensive lifestyle audit, and the full suite of baseline investigations – arranged and coordinated for you (All-Inclusive), or self-arranged if you prefer. All results are reviewed before your first consultation takes place.

Initial Consultation
Up to two hours. Every baseline result integrated. A bespoke clinical strategy covering metabolic health, body composition, cardiovascular risk, hormonal optimisation, nutrition, training direction, and sleep. Followed by a written clinical summary.

Bi-weekly Strategy Reviews
Every two weeks, a 60-minute consultation to review compliance, assess progress, adjust course, and address emerging data. Each session produces a written clinical summary – the authoritative record of recommendations made. Between sessions, priority asynchronous messaging via encrypted channel keeps your physician accessible without waiting for the next appointment.

Milestone reassessment
At three- to six-month intervals, the full investigative cycle repeats. Bloods, DEXA, biological age – the same rigour applied to the same data points, so your trajectory is measured, not guessed.
A note on scope
This service is deliberately narrow.

I do not replace your GP. I do not manage acute illness. I do not provide emergency care, and I am not a therapist, personal trainer, or dietitian.

What I provide is the clinical oversight layer that sits above all of these: the physician who reads everything, integrates everything, and provides you with a clear, evidence-based clinical strategy – one you can act on directly and share with your existing health professionals where relevant.

Where your data indicates a need for prescribing within the UK, I can do so – within defined scope and in compliance with GMC and HIS standards. Where it indicates a need for specialist investigation, I issue the referral. Where it indicates a need for a change in training or diet, I provide the clinical rationale – and you take it to the right professional.

This is a service for adults who are serious about their long-term physiological performance and prepared to commit to the process. It is not for everyone. The free onboarding videocall exists precisely to determine whether it is for you.

What is this service?
A remote, consultant-led private medical advisory for UK-domiciled adults aged 18 and over. It combines deep-data baselining – advanced blood panels, body composition, cardiorespiratory fitness, biological age, continuous glucose monitoring, nutrition audit, and wearable data – with longitudinal clinical oversight delivered via regular videocall consultations.
What this service is not
This is not emergency care, not a GP replacement, not a therapist, personal trainer, or dietitian. It does not treat patients under 18, those who are currently pregnant, those with active malignancy under treatment, or those experiencing acute psychiatric crisis. It does not provide out-of-hours cover.
Who is this for?
Adults who are serious about understanding and optimising their long-term physiological performance. Typically professionals aged 35–65 who want clinical-grade oversight applied to their health with the same rigour they apply to their careers. You must be UK-domiciled and retain your own GP and local healthcare infrastructure.
How are consultations delivered?
All consultations are delivered remotely via secure videocall (Google Meet – encrypted in transit, Workspace-managed, two-factor authentication). There are no in-person consultations. Priority asynchronous messaging between sessions is available via end-to-end encrypted platforms (Signal is recommended).
Can you prescribe?
Yes, within defined scope. Remote prescribing is available for UK-domiciled service users only, via GPhC-registered pharmacies. No controlled drugs (Schedules 1–5). No weight-management pharmacotherapy (signposting and ancillary monitoring only). All prescribing complies with GMC Good Practice in Prescribing (2021) and the High Level Principles for Good Practice in Remote Consultations and Prescribing.
How is my data handled?
All clinical data is processed in compliance with UK GDPR and stored securely within Google Workspace (UK/EEA data residency, AES-256 encryption, two-factor authentication). Clinical data is pseudonymised using a unique service user identifier, with the mapping held separately in secure storage. Full details are available in the Advisory Privacy Notice. What does it cost?
Pricing is discussed during the free onboarding videocall, once suitability has been confirmed. Both standalone diagnostic strategy engagements and longitudinal retainer arrangements are available. All fees are VAT-exempt (Schedule 9, Group 7, VATA 1994) and strictly non-refundable once payment is received.

If both parties are satisfied, we proceed to clinical intake. This service is currently pending registration with Healthcare Improvement Scotland.

My clinical background is in acute medicine – the operating theatre, the intensive care unit, the resuscitation room. I spent years managing patients at their most physiologically vulnerable. This service exists because I became convinced that the same rigour applied upstream – years or decades before the acute event – changes outcomes in ways that reactive-only medicine cannot.

I practise as a Consultant within NHS Greater Glasgow & Clyde. This private advisory is delivered alongside my NHS commitments, with a deliberately small caseload to ensure that every service user receives the depth of attention this work demands.
