1. Service Overview

Dmitry Sokolov MD – Private Medical Advisory (Dmitrijs Sokolovs, Sole Trader) operates a remote, Consultant-led Independent Medical Agency (telemedicine) service delivered from Scotland. The service is registered with Healthcare Improvement Scotland (HIS) as an Independent Medical Agency under the Public Services Reform (Scotland) Act 2010. HIS Registration No.: [pending]. Service users may verify the service's registration at healthcareimprovementscotland.org.

The service provider is Dr Dmitrijs Sokolovs (GMC 7055978), a GMC-registered Consultant physician who provides deep-data baselining, diagnostic interpretation, longitudinal performance monitoring and management. This is supported by regular written clinical summaries and secure asynchronous clinical correspondence.

This service provides medical oversight focused on physiological baselining, habit architecture, and performance optimisation, utilising evidence-based nutrition planning, clinical exercise prescription, sleep optimisation protocols, and structured monitoring.

This approach leverages multi-dimensional objective baselining – including advanced blood panels, DEXA scanning, cardiopulmonary exercise testing (CPET), GlycanAge biological age assessment, continuous glucose monitoring (CGM), continuous nutrition audit (Cronometer), and tracker device output – to create bespoke, evidence-based strategies.

Where clinically indicated, the Advisor may issue referrals to regulated private healthcare facilities and UKAS-accredited diagnostic laboratories within the UK. The service provides remote prescribing within the UK, where appropriate and clinically indicated (see Section 5).

The service is complementary in nature and does not replace the service user's NHS GP, local primary care, or specialist services. Service users must retain and engage their local healthcare infrastructure for routine screening, acute illness management, urgent concerns, and ongoing GP/specialist care.

All fees charged by this service are exempt from VAT under Schedule 9, Group 7 of the Value Added Tax Act 1994.

2. Delivery Tracks

Track A (All-Inclusive): The Advisor orders and manages relevant private diagnostics.

Track B (Self-Arranged): The service user self-arranges diagnostics via their own preferred private providers or the NHS.

Regardless of track selected: the Advisor acts as a treating physician; the service is delivered remotely from Scotland for service users domiciled in the UK; service users are expected to retain and engage their usual primary care and specialist services; this service does not provide urgent triage or emergency care; prescribing is available where clinically appropriate (see Section 5).

4. Longitudinal Medical Advisory (Retainers)

For continuous clinical oversight, the Advisor integrates full diagnostic baselines into an ongoing medical and lifestyle strategy.

Encrypted messaging is for brief, non-emergency clinical communication. It is not a substitute for your bi-weekly consultation and is not an emergency channel. In a medical emergency, contact 999 immediately.

5. Financial & Operational Policies

Non-Refundable Fees: Standalone engagement fees are strictly non-refundable once payment has been made, as clinical time is allocated during the onboarding assessment. Retainer fees are strictly non-refundable once payment has been received, as the fee covers the Pre-Retainer Phase and full duration of clinical time.

Emergency Care Exclusion: This is a scheduled advisory service. In a medical emergency, service users must immediately contact local emergency services (999).

Prescribing: Where clinically appropriate, the Advisor may issue UK prescriptions within the defined scope of this service and in accordance with applicable UK regulatory requirements. This service does not prescribe controlled drugs (Schedules 1–5). This service does not prescribe, initiate, or take clinical responsibility for weight-management pharmacotherapy. Where a service user is already prescribed such medication by an independent provider, this service may provide ancillary monitoring support within the advisory scope of this practice.

Rescheduling and Cancellation: Bi-weekly sessions may be rescheduled with a minimum of 48 hours' notice. Late cancellations (fewer than 48 hours) and non-attendance count as a delivered session against the retainer. In all such cases, a brief written clinical update will be provided to maintain continuity.

Practitioner Absence: Planned absence: 14 days' notice. Unplanned absence: notification as soon as reasonably practicable. Missed sessions rescheduled at earliest opportunity. No out-of-hours or emergency cover during any period of absence.

GP Communication: Service users are encouraged to consent to sharing of findings with their NHS GP. Limited or no disclosure is the service user's choice; implications are explained in the Clinical Intake, Consent Agreement & Terms of Business (Document 20).

Record Retention: Clinical records retained for a minimum of 10 years from the date of last contact, or longer where there is an open complaint, adverse incident, regulatory inquiry, or actual/anticipated claim.

6. Service Exclusions

  • Persons under 18 years of age
  • Persons in the active phase of malignancy/cancer diagnosis under treatment
  • Persons who are pregnant
  • Persons presenting with acute psychiatric illness, active major depressive episode, suicidal ideation, or other acute mental health crises
  • Persons presenting with a medical emergency or acute life-threatening condition – directed immediately to 999/A&E
  • Persons seeking a replacement for primary care or specialist NHS care

7. Your Rights as a Service User

In accordance with the Health and Social Care Standards: My Support, My Life (Scottish Government, 2017), you have the right to: withdraw consent and disengage at any time; access your clinical records (Subject Access Request, responded to within one calendar month); request rectification of inaccurate data; raise concerns without detriment; involve a representative of your choosing; and receive clear, honest information about your care.

3. Who Data is Shared With

Data is shared only with approved subprocessors operating under DPAs or appropriate contractual and technical safeguards, as detailed in the Data Protection Impact Assessment (Document 21), available to service users on request.

5. Your Rights

6. Complaints

7. Breach Notification

Any notifiable breach will be reported to the ICO within 72 hours and affected service users notified without undue delay in accordance with UK GDPR Article 34.


Complaints Handling Policy +

1. Policy Statement and Aims

Dmitry Sokolov MD – Private Medical Advisory welcomes complaints as an opportunity to improve. All complaints are handled fairly, promptly, and respectfully. The complainant is kept informed throughout the process. Complaints may be submitted by the service user or by a family member, carer, advocate, or representative acting on their behalf. Raising a complaint will not affect the complainant's care or standing as a service user.

Healthcare Improvement Scotland is the regulator for independent healthcare services in Scotland and can accept complaints at any time.

2. How to Make a Complaint

3. Acknowledgement

All complaints are acknowledged in writing within 3 working days of receipt, confirming receipt and providing an expected timeline for response.

4. Investigation and Response

A thorough investigation is completed within 20 working days. Where this is not possible, the complainant is informed of the delay and a new target date provided. The investigation outcome, apology where appropriate, explanation, and corrective actions are communicated in writing.

Where Healthcare Improvement Scotland conducts an investigation into a complaint about this service, Dr Sokolovs will cooperate fully with the investigation, provide access to all relevant records, and comply with any requirements or recommendations arising from the complaint investigation report.

5. Complaining to Healthcare Improvement Scotland

Healthcare Improvement Scotland is the final stage for complaints about independent healthcare services in Scotland. If you wish to raise a concern about how HIS conducted the investigation of your complaint, you may do so via his.ihcregulation@nhs.scot or his.comments@nhs.scot.

6. Monitoring

All complaints are logged in the Complaints Register. The number and nature of complaints are reviewed annually as part of the clinical governance review. Complaint records are retained in accordance with the service's record retention policy (minimum 10 years from the date of last contact).


Duty of Candour – Annual Statement

The annual Duty of Candour statement will be published here from March 2027, in accordance with the Duty of Candour provisions of the Health (Tobacco, Nicotine etc. and Care) (Scotland) Act 2016 and the Duty of Candour Procedure (Scotland) Regulations 2018.
